How do I participate in the Fusion Friends referral program?
To join, simply sign up and receive your unique 5-digit referral code. Share your referral code with businesses or individuals who may benefit from FusionForm’s services. When they book a free discovery call with us, they must enter your referral code in the designated box.
What rewards can I earn through the referral program?
You will earn a 5% commission via a prepaid Visa gift card  when your referred client signs a design project contract and the project is completed. The reward is capped at $100 per project and $500 per individual/household annually.
Can I share my referral code on social media or in public forums?
No, referral codes are intended for personal use only. Public sharing or advertising of referral codes is prohibited and may result in disqualification from the program.
When and how will I receive my referral bonus?
Once the referred client’s project is completed and paid in full, your bonus will be issued as a Visa prepaid card to the email address provided during your program enrollment. Payment is typically processed within one week of the final project payment.
Are there any restrictions on who I can refer?
Yes, referred clients must be new to FusionForm and resides in the United States. The referral code must be entered during their discovery call booking, as it cannot be added retroactively.

Terms and Conditions

Effective Date: 12/30/2024

  1.  Eligibility: The referral program is open to all clients and individuals who are residents of the United States. International participants are not eligible. Referral bonuses are capped at a total of $500 per individual and/or household.

  2. Referral Submission: Referrals must be submitted by the referred individual using the referral code box when signing up for a consultation call. Referral codes are intended for personal use only and may not be shared publicly or advertised in any form. FusionForm reserves the right to disqualify any participant found violating this condition. Referral codes must be entered at the time of consultation booking and cannot be added retroactively. The referrer will receive a unique 5-digit referral code upon enrollment in the program.

  3. Payment Process: Referrers will receive a payment of 5% of the total amount paid by the referred client, capped at $100 per project. Payments will be issued via Visa prepaid cards, sent to the email address provided during program enrollment. Payments will be made within one week of receiving the final payment for the referred client’s project. FusionForm is not responsible for failed delivery of Visa prepaid cards due to incorrect or invalid email addresses provided by the referrer.

  4. Referral Limit: There is no limit to the number of referrals a participant can make; however, total earnings from the referral program are capped at $500 per individual and/or household to comply with tax regulations. The cap applies regardless of multiple email addresses or family members participating.

  5. Service Inclusion: All services provided by FusionForm are eligible for the referral program. The referral bonus is based solely on the total payment received from the referred client. Referral bonuses will not be awarded for non-service offerings, such as products or one-off consultations.

  6. Program Termination: FusionForm reserves the right to terminate the referral program at any time with 30 days’ notice. Referral bonuses for pending projects during this period will still be honored. Participants will be notified of any program termination via email.

  7. Referral Bonus Conditions: The referral bonus will only be awarded once the referred client has paid in full for their project. Bonuses will not be awarded if the referred client cancels their project or requests a refund after payment. FusionForm reserves the right to delay or cancel referral bonuses in cases of payment disputes or chargebacks.

  8. Payment Finality: Once a referral bonus is paid, it is final and cannot be reversed unless issued in error.

  9. Tax Compliance: FusionForm is not responsible for ensuring participants comply with applicable tax laws. Participants are solely responsible for reporting and paying any taxes related to referral bonuses they receive. FusionForm will not be held liable for any false or inaccurate information provided by participants regarding their tax obligations. If required by law, FusionForm may issue tax forms (e.g., 1099-NEC) for referral bonuses exceeding applicable thresholds. Participants are responsible for providing accurate information to facilitate compliance.

  11. Modifications: FusionForm reserves the right to modify these terms and conditions at any time. Modifications will apply only to future referrals, not retroactively. Participants will be notified of any changes via email.
